Premises licence holders, designated premises supervisors (DPS) and personal licence holders are required under the Licensing Act 2003 to notify us as soon as possible of any changes to names or addresses. 

The notification of change details - premises licence form should be used for changes to:

• trading name
• registered company name (where the company registration number remains the same)
• the address of the registered company (as displayed by Companies House)

A fee is payable when making these changes.

The surrender a premises licence for should be used when surrendering a:

• premises licence
• club premises certificate
• personal licence

There is no fee when making these changes.
